CANCELLING AN APPOINTMENT

 

Prices

With effect from 1st August 2022, my charges will be £40.00 for initial assessment and treatment and any onging treatment sessions.  You can pay by cheque, cash or bank transfer.

 

Cancellation Policy

ask that you show me and other patients consideration by calling at least 24 hours prior to your appointment, where possible, if you are unable to attend.  This will allow me the opportunity to offer that appointment to another patient.


Failure to give the 24 hours’ notice may result in a charge for the appointment, at my discretion. If you are claiming for treatment through your medical insurer, they may not pay the cancellation charge and therefore you may be liable for this payment.  I look forward to your understanding and co-operation.

An overview of how I collect and use Patient information

 

This is an overview of:

  • the types of information I collect about you
  • how I collect and use it
  • who I might share it with
  • the steps I take to make sure it stays private and secure
  • your rights to your information.

 

The information I collect

I collect information about you from different places including:

  • directly from you
  • from a third party acting on your behalf e.g. insurers
  • when I generate it myself
  • from other healthcare professionals.

 

I only collect your information in line with relevant regulations and law and this will relate to assessments and treatments you request, have currently requested or have used in the past. You’re responsible for making sure you give me accurate and up to date information. If you provide information for another person, you’ll need to tell them how to find the Privacy Notice and make sure they agree to me using their information for the purposes set out in it.

 

How I’ll use your information

I use it to assist me in my assessments, treatments and programmes you’ve requested and other purposes e.g.:

  • to confirm your identity and address
  • to understand your health history and limitations
  • to carry out your instructions
  • to improve my treatment and services

 

Who I can share your information with

I may share your information with other health care professionals I work in partnership with. I may also share your information with others e.g. regulators or insurers.

 

How long will I keep your information

I will keep your information for as long as you have a relationship with me. After it ends I will keep it where I may need it for my legitimate purposes e.g. to help me respond to queries or complaints, or for other reasons e.g. responding to requests from regulators.

 

Your rights

You have a number of rights relating to your information e.g. to see what I hold, to ask me to share it with another party, ask me to update incorrect or incomplete details, to object to or restrict processing of it, to make a complaint etc.
